Why are there so many bad choices in health insurance?

I was privileged to serve as Medical Director for two health plans created under the Affordable Care Act: Health Republic Insurance of New York, and Health Republic Insurance of New Jersey. Formed as Community Owned and Operated Health Plans (COOP), they represented two of the 23 COOPs designed to increase competition. The majority of these COOPs failed due to the withdrawal of federal funding at the end of the first year. Notwithstanding the many challenges of the New York and New Jersey COOPs, these plans would have succeeded if that funding had remained available.
